We Pause to Examine the Large Joint Market Ahead of the Conclusion of Earnings Season BY JOHN CHOPACK, JULY 19, 2004

With DePuy, Stryker and Biomet reporting during the last three weeks, this is a good time to identify specific topics imperative to the remainder of the sector's earnings releases over the next several weeks.

Large Joint Companies Earnings 2Q:04 Reports to Date
2Q:04 1Q:04
Hip Knee Overall Hip Knee Overall
DePuy 10% 15% 9% 9% N/A 8%
Biomet 10% 18% 14% 8% 12% 11%
Stryker 9% 18% 12% 17% 18% 17%
Mean 10% 17% 12% 11% 15% 12%

All total to date, the 2Q:04 large joint and overall orthopedic market resembles the 1Q:04. There are some factors that should be watched closely, including:

  • Stryker has slowed down in hip sales from 17% during the 1Q:04 to 9% during the 2Q:04. Although the 2Q:04 represented the anniversary of the introduction of the Trident ceramic-on-ceramic hip system, we were not expecting such a significant slow down. Trident unit sales now represent 32% of all acetabular liner sales, and are clearly slowing in growth.
  • This is the second quarter in a row that DePuy posted weak orthopedic growth compared to the rest of the market. However, knee sales remain strong at 15%.
  • Biomet's growth is currently outpacing the rest of the market driven by strong knee sales.

Additional points to consider as the rest of the sector reports:

  • Is orthopedic implant pricing increases slowing? Stryker reported that U.S. pricing increased by 3% during the 2Q:04 this significantly lower than the 5% increase in pricing during 2003.
  • Pricing in Japan is expected to be 4-6% lower than it was a year ago.
  • Penetration of alternate bearing surfaces increased during the previous 4 quarters. Ceramic-on-ceramic, metal-on-metal and highly cross-linked poly articulating materials have driven hip growth. Are we set for a slow down in penetration of these products which would have a negative impact on the overall market?
  • When does the industry anticipate launching a mobile bearing knee, provided a down classification is approved by the FDA?
